Signing up to play on a North Valley Soccer Club team is as simple as 1-2-3!

1.  Complete the one-page Player Registration Form and attach the required documentation.

2.  Complete the Medical Release and have your signature notarized.

3.  Complete the Code of Conduct and Concussion Forms.

All paperwork must be complete and submitted to Jane Fallon, Registrar, before your child’s registration can be processed.  The Registrar also must view a copy of an original birth certificate prior to processing registrations.  Teams will arrange times for this viewing to take place.  Individual families may contact Jane Fallon to make arrangements for individual viewing of original birth certificates.

Registrations received by Monday will be processed by Friday.  You may mail them to NVSC, 1729 E Potter Dr, Phoenix, AZ  85024 or simply scan and email them to Jane.Fallon@nvsoccer.org.
